dangerous, unpredictable transmission behavior Violent Downshifting Sudden jerks and Deceleration and Power Loss: heavy jerking loud clunks gears making noises following the forced downshift, the truck will experience a massive, unexpected drop in speed, sluggish and difficult maintaining speed like limp mode driving in normal speeds then loss of power engine stalling in the middle of rush hour traffic The Orange Wrench Light and check engine light will come on with codes p0720 I went did the reprogramming as recommended Ford dealer performed Safety Recall 26S28, which immediately aggravated the transmission failure and left the vehicle completely disabled. in the highway the dealer's recall software update directly caused a total transmission failure the next day, and i feel unsafe driving the vehicle due to the severe unprompted downshifting risks outlined in NHTSA Campaign 26V237 Felt unsafe to drive truck and also it became undrivable and had to have it towed to the Ford dealership that did recall.

On May 10, 2026, my 2016 Ford F-150 (6R80 transmission) went into limp mode and displayed a wrench warning light while driving. The truck lost normal transmission function and would not shift properly, creating a hazardous situation in traffic. The PCM has stored active Output Shaft Speed sensor codes P0720 and P0722, confirmed by a Paul Miller Ford IDS diagnostic scan, indicating a failing molded leadframe inside the transmission. When the OSS signal drops out at road speed, the PCM can command a sudden unintended downshift into a low gear, causing abrupt deceleration and potential rear-wheel lockup and loss of control — a serious safety hazard. Ford Recall 26S28 (26V-237) / Program 26N04 for 2015–2017 F-150s only covers a PCM software recalibration for the Transmission Range Sensor codes (P0705/P0706). It does not replace the leadframe hardware for the OSS codes (P0720/P0722) my truck is actually throwing. Ford's system auto-rejected the claim, and a dealer SSSC prior-approval request was denied. I am being told to pay out of pocket for a known defective part. For 2014 trucks, Ford replaced this same leadframe for free under Recall 24S37 when P0720/P0722 were present. My 2016 has the same transmission, same part, and same failure, but the current recall remedy is software only. I'm filing so NHTSA has documentation that the OSS leadframe failure is occurring on 2015–2017 F-150s and to request the remedy be expanded to cover leadframe replacement. No crash or injury occurred; the defect was confirmed by a Ford dealer.

Ford recently did a software update for a unexpected downshift recall that had previously happened to me while doing 60 miles an hour in my vehicle downshifted into first gear in my truck went sideways because the rear end locked up. Drove my vehicle home and took it out today and while doing 45 miles an hour it downshifted into first gear locking up the rear wheels again. Thankfully, this was at a lower speed so I was able to control the vehicle easily.. the turnpike speed limit in Maine is 70 miles an hour so if this happened at that speed in traffic, I’m almost positive there could have been a serious accident. So their update did not fix this issue.

The contact owns a 2016 Ford F-150. The contact stated that while driving at an undisclosed speed, the transmission unexpectedly downshifted. The contact stated that the power train warning light and several other unknown warning lights were illuminated. The contact was notified that the VIN was included in NHTSA Campaign Number: 26V237000 (Power Train). The vehicle was taken to a dealer where it was diagnosed; however, the diagnostic result was unknown. The vehicle was repaired; however, the failure persisted, and the warning lights were flashing. The vehicle was taken to an independent mechanic where it was diagnosed and determined that an unknown transmission part and the engine needed to be replaced. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was unknown.

The contact owns a 2016 Ford F-150. The contact stated that while driving at various speeds, the transmission unexpectedly downshifted and became stuck in a lower gear before shifting into the intended gear. The powertrain warning light was intermittently illuminated. The vehicle was taken to the dealer, where DTC: P0705, P0706, P0707, and P1702 were retrieved, indicating a powertrain control module (PCM) and leadframe failure. The vehicle was not repaired because the warranty had expired the day prior. The contact later received notification of NHTSA Campaign Number: 26V237000 (Power Train), and the vehicle was taken to the dealer, where the PCM software update was completed. The contact was advised to drive the vehicle for an extended period and return to the dealer to determine if the leadframe needed to be replaced. The failure persisted while driving, and upon returning to the dealer, DTC: P0717 and P0715 were retrieved. The dealer submitted a claim to the manufacturer, but coverage for the leadframe replacement was denied because the diagnostic trouble codes did not match the recall specifications. The contact stated that the PCM software update erased the initial diagnostic trouble codes, preventing the leadframe replacement under the recall. The vehicle was not repaired. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The failure mileage was approximately 81,000.

I went to start my truck and the brake went all the way to the floor. I had no braking power and it had to be towed back home over an hour away. There is a recall on 2016 Ford F150 with the 3.5 Eco Boost engine which I have. This recall involves a seal in the master cylinder failing and losing braking power and affecting the brake booster. Referencing recall 20S31. I went to Ford and they told me the recall did not apply to this specific truck due to a different batch of parts. This truck is now having the exact problem as the specific ones which have already been recalled. I believe the recall should be expanded. This occurred shortly after driving an hour in the rain with my kid in the back. It could have easily killed us on the highway with no brakes. We had no idea this recall was in effect. It’s the last time I’m buying a Ford. I want to feel safe with my kids in the back and Ford does not seem to care.

On XXX I lost all brakes in my 2016 Ford F-150 while towing a fishing boat. I had just decended a steep grade on a busy downtown street when I received an alert that I was low on brake fluid. The next pump of the brake went to the floor. I received an audible alarm. Had this occured on the steep grade, this could have been a very serious accident. I understand the loss of brakes was due to failure of the master cylinder just and as described by the open recall on my vehicle. I added a quart of brake fluid to an empty resevoir, tried to pump the brakes up and the fluid vanished. I could not find any evidence of a leak. Clearly the fluid is in the brake booster, which is also replaced under this recall. I had the vehicle delivered to the closest dealership, Pierre Ford of [XXX] on Monday the XXX. The service advisor tod me he would need ot for two days. Four days later, the advisor text me to report that parts are not available, and that he would let me know when they were. I asked him to make sure that a VOR report had been submitted so that my repair repair parts could be expidited by Ford Corporate. He assured me the VOR had been placed. Today, on XXX, I called Customer Care who verified my recall eligability, and also informed me that no VOR had been requested against my VIN. I have found genuine ford parts for the repair of my vehicle in many Ford facilities in the nation. I dont feel that Ford is working very hard to get my only vehicle back on the road INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The message “Service Vehicle” was displayed and the wrench symbol was illuminated. The vehicle was taken to the dealer, who confirmed that the vehicle was included in NHTSA Campaign Number: 26V237000 (Power Train) and the vehicle was repaired. The contact stated that approximately 5 miles after retrieving the vehicle, the failure reoccurred. The contact called the dealer and confirmed that the recall had been closed because the recall repair was previously performed, and that any further inspections or repairs would be out-of-pocket. The contact took the vehicle to a certified mechanic, and the vehicle was repaired. The contact was uncertain of the repair but confirmed that the failure had not occurred since the repair. The manufacturer was not informed of the failure. The failure mileage was unknown.

On June 18, 2018, the Office of Defects Investigations (ODI) opened a Preliminary Evaluation (PE18-008) to investigate complaints alleging post-crash fires at the B pillar in the area of the seat belt pretensioners on MY 2014-2018 Ford F-150 Regular Cab, and Super Crew Cab Vehicles. On August 24, 2018, Ford Motor Company recalled (FSA 18S27 - NHTSA #18V568) the affected population. Ford's investigation found that some front seat belt pretensioners may generate sparks during deployment in excess of Ford's specification. Sparks in the pretensioner exhaust gases inside the lower portion of the B-pillar may subsequently ignite combustible components contained in the B-pillar, such as insulation or carpet, and may cause a subsequent vehicle fire. ODI also requested relevant information from other manufacturers to assess the potential scope of post-crash pretensioner fires. Ford will notify owners and require dealers to remove B-pillar insulation material and wiring harness tape, and install heat resistant tape, free of charge. Summary
Ford Motor Company (Ford) is recalling certain 2015-2017 F-150 vehicles. A loss of signal between the transmission range sensor and the powertrain control module can cause the transmission to unexpectedly downshift into second gear.
Safety Consequence
Unexpectedly downshifting into second gear may result in a loss of vehicle control, increasing the risk of a crash.
Remedy
Dealers will update the powertrain control module software, free of charge. Owner notification letters were mailed July 24, 2026. Owners may contact Ford customer service at 1-866-436-7332. Ford Motor Company (Ford) is recalling certain 2016-2017 Ford Expedition, Lincoln Navigator, and 2016-2018 F-150 vehicles equipped with 3.5L Ecoboost engines. The brake master cylinder may allow brake fluid to leak from the front wheel circuit into the brake booster.
Safety Consequence
A loss of brake fluid may result in a change in brake pedal travel, increased pedal effort and reduced front brake function, requiring greater effort and distance to stop the vehicle. These conditions can increase the risk of a crash.
Remedy
Dealers will replace the brake master cylinder. If the master cylinder is leaking, the brake booster will also be replaced. Repairs will be performed free of charge. An interim owner notification letter notifying customers of the safety defect were mailed on April 29, 2022. Owner notification letters were mailed April 29, 2022. Summary
Tenneco Automotive (DRiV) is recalling certain MOOG K500359 ball joints, for aftermarket installation on 2015-2018 Ford F-150 trucks. The ball joint may have a misaligned dust boot and may be missing alignment hole and installation warning tag, which may lead to improper alignment of the ball joint.
Safety Consequence
An incorrectly installed ball joint may cause the ball joint stud to break, which can compromise front wheel control, increasing the risk of a crash.
Remedy
DRiV will notify owners, and dealers will inspect, and if necessary replace the ball joint, free of charge. Owner letters were mailed May 5, 2021. Owners may contact Tenneco Automotive customer service at 1-877-489-6659.

Ford Motor Company (Ford) is recalling certain 2015-2016 F-150 pick-up trucks equipped with 3.5L GTDI engines and block immersion heaters that were potentially relocated from their originally installed location. The incorrect location could put the heater too close to hot engine components, possibly resulting in a damaged cable.
Safety Consequence
A damaged cable could cause a resistive short, increasing the risk of overheated or melted wiring, increasing the risk of a fire.
Remedy
Ford will notify owners, and dealers will inspect the block heater's location and its cable, replacing components as necessary, free of charge. If the replacement parts are not available, dealers will disable the existing block heater cable from being used until parts become available. The recall began April 9, 2020. Ford Motor Company (Ford) is recalling certain 2015-2019 Ford F-150 and 2017-2019 Ford F-250 Super Duty, F-350 Super Duty, F-450 Super Duty, and F-550 Super Duty pick-up trucks equipped with an engine block heater and previously remedied under recall 18V894. During the previous repair, the block heater cable's splice connector may have been damaged.
Safety Consequence

Water and contaminants may get into the block heater cable's splice connector causing corrosion and damage.
Safety Consequence
When the heater is plugged in, prolonged corrosion could cause a resistive short, increasing the risk of overheated or melted wiring which can increase the risk of a fire.

Safety Consequence
A vehicle fire could result if materials ignite inside the vehicle.

Summary
Ford Motor Company (Ford) is recalling certain 2016 F-150 and Explorer vehicles. Loose power seat adjuster pivot bolts may cause the front seat cushions to detach and not properly restrain the seat occupant in the event of a crash.
Safety Consequence
In the event of a crash, a seat occupant that is not properly restrained has an increased risk of injury.
Remedy
Ford will notify owners, and dealers will inspect the tightness of the power seat track upper pivot link bolt, either applying threadlocker or replacing the lift link and hardware, free of charge. Interim letters are expected to go out January 15, 2018. Owners will receive a second notice when the remedy becomes available. Owners may contact Ford customer service at 1-866-436-7332. Ford Motor Company (Ford) is recalling certain 2015-2017 Ford F-150, and 2017 F-250, F-350, F-450, and F-550 Super Duty trucks. The doors on these vehicles may have a bent or kinked actuation cable or the door latches may freeze after water gets into the latch. Either condition can result in the doors opening while driving although they appear to be closed.
Safety Consequence
A door that opens while driving can increase the risk of injury.
Remedy
Ford will notify owners, and dealers will inspect the door latch actuation cables and repair if necessary, and install water shields over the door latches, free of charge. Summary
Ford Motor Company (Ford) is recalling certain 2015-2017 F-150, and 2017 F-250 and F-350 trucks equipped with a crew cab. The left rear inflatable seat belt buckle assembly may be inadequately attached to its mounting bracket. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 210, "Seat Belt Assembly Anchorages."
Safety Consequence
If the left rear inflatable seat belt buckle separates from it's mounting bracket, the seat occupant may not be adequately restrained, increasing their risk of injury.
Remedy
Ford will notify owners, and dealers will inspect the seat belt assembly, replacing the attaching rivet as necessary, free of charge. Interim notices informing owners of the safety risk were mailed October 18, 2017. Owners will receive a second notice when the remedy becomes available. Ford Motor Company (Ford) is recalling certain 2015-2017 F-150, and 2017 F-250, F-350, F-450, and F-550 vehicles originally produced with black or body colored exterior door handles that may have been subsequently equipped with a chrome exterior door handle cover accessory kit purchased from a Ford dealer. These accessory door handle covers may cause the doors to unlatch in a side impact collision. As such, these vehicles may fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 206, "Door Locks and Door Retention Components."
Safety Consequence
If the doors unlatch and open in a side impact collision, the occupants have an increased risk of injury.
Remedy

The seat back frame may have insufficient welds which may not meet the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 202, "Head Restraints" and 207, "Seating Systems."
Safety Consequence
The seat back may not adequately restrain the occupant during a crash, increasing the risk of injury.
Remedy

Summary
Ford Motor Company (Ford) is recalling certain model year 2016 F-150 trucks manufactured April 27, 2015, to November 22, 2015, and equipped with Multi-Contour Seats (MCS). The front passenger seat Occupant Classification System (OCS) that calibrates the air bag deployment level may incorrectly classify an adult as a child when the seat massage feature is activated.
Safety Consequence
If an adult is incorrectly classified as a child, the passenger frontal air bag will not deploy during a crash, increasing the risk of injury.
Remedy
Ford will notify owners, and dealers will update the Occupant Classification System Module (OCSM) with new software, free of charge. The recall began on March 11, 2016. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 16S05.
